What the ER Wants You to Know Before the Next Heat Dome
Six hours into a heat emergency, our waiting room fills in a pattern you could set a clock by. The roofer who felt fine until lunch. The 82-year-old whose window unit died two days ago and did not want to bother anyone. The teenager from afternoon practice. I have been an ER nurse for eleven years, and heat waves are the most predictable disaster we treat — which also makes them the most preventable.
This week it is Philadelphia's turn — the city declared a heat emergency as hospitals strain — but the same pattern plays out in every ER under the heat dome. And almost every bad outcome I have seen traces back to the same twenty minutes: the window where heat exhaustion becomes heat stroke and nobody nearby knew the difference.
Two questions that decide everything
Heat exhaustion looks like this: heavy sweating, weakness, nausea, maybe a pounding headache — but the person is still themselves. They answer questions. They know where they are. That person needs shade, water, and aggressive cooling right now, and they will usually be okay. Heat stroke looks different. Confusion sets in, speech gets strange, and — this is the part that fools people — the skin can turn hot and dry, because the body's cooling system has failed. Someone who has stopped sweating in extreme heat is not getting better. That is a 911 call, and you cool them with everything you have while you wait.
The best short explanation of this I have seen anywhere was not in a nursing journal. It was a physician on TikTok breaking down heat-dome survival in under a minute — including the fan rule that surprises almost everyone: above roughly 95°F indoors, a fan alone can start blowing heat into you instead of off of you.
His checklist matches what we tell discharged patients almost word for word: move outdoor work out of the 1–6 p.m. window, pair fans with cool showers or skip them when it is hotter than about 95°F inside, treat libraries and malls as free cooling centers, and do a daily buddy check on older neighbors, outdoor workers, and anyone without AC. I will add one thing from the triage desk: the patients who had someone checking on them come in hours earlier — and hours are everything.
This is not just a weather story
Zoom out and the same week looks like this: record-breaking heat in Europe has killed dozens and is straining power grids across the continent. That last part matters more than people realize. In a heat dome, air conditioning is life-support equipment — and it only works if the grid holds. When the power fails on the hottest day of the year, my waiting room is where that failure ends up.
